February 2027 is the anticipated opening timeframe, not a confirmed appointment date. Planned services, pricing, payment methods, and availability may change before appointments open.

Submitting either form requests opening updates or a partnership response. It does not confirm a visit, establish a nurse–client relationship, or guarantee suitability or service-area availability. Suitability, location, fees, and availability will be discussed before confirming a future visit.
